Title: Consideration and possible approval of an application for a Gaming license for Pizzajeep Corporation, dba Boulevard Pizza located at 1076 North Rock Blvd., Sparks, NV submitted by Mr. Anthony Koehler.
Petitioner/Presenter: Mr. Anthony Lorenz Koehler resides at 8180 Seneca Drive, Reno, NV./Chief of Police Brian Allen
Recommendation: It is recommended that Council approve Mr. Anthony Lorenz Koehler application for a Gaming license contingent upon the location being inspected and approved by every city, county, district, and state agency having jurisdiction over the matter and upon fingerprints being returned from the FBI/CJIS that do not reflect any disqualifying arrests/convictions.
Financial Impact: $30.00 x 6 = $180.00 quarterly. There will be 6 Bally Game Maker Multi denomination coin operated machines.

Background:

Mr. Anthony Lorenz Koehler is applying for a Gaming License for Pizzajeep Corporation, dba Boulevard Pizza at 1076 North Rock Blvd., Sparks, Nevada.  Mr. Anthony Lorenz Koehler has been working for his father at Boulevard Pizza, a family owned business, for 17 years.  He was a Staff Member at Boulevard Pizza from 1998-2004 when his position changed to Manager and has been a Manager from 2004 to present.  Mr. Anthony Lorenz Koehler’s father, Chris Koehler, is transferring ownership to him as of 01/01/16.



Analysis:

Mr. Anthony Lorenz Koehler has complied with all existing ordinances and requirements.



Alternatives:

Council may approve or disapprove this application.



Recommended Motion:

I move to approve Mr. Anthony Lorenz Koehler’s application for a Gaming license for Pizzajeep Corporation, dba Boulevard Pizza at 1076 North Rock Blvd., Sparks, Nevada contingent upon the location being inspected and approved by every city, county, district, and state agency having jurisdiction over the matter and upon fingerprints being returned from the FBI that do not reflect any disqualifying arrests/convictions.
