Title: Consideration and possible approval of an application for a Gaming License for Jett Gaming LLC db at Safeway #4626, located at 2808 Vista Boulevard, Sparks, NV 89434, submitted by Mr. Forrest William Bietz.

Recommendation: It is recommended that Council approve Mr. Forrest William Bietz’s application for a Gaming license contingent upon the location being inspected and approved by every city, county, district, and state agency having jurisdiction over the matter and upon fingerprints being returned from the FBI/CJIS that do not reflect any disqualifying arrests/convictions.
Financial Impact: $210 quarterly in total. There will be 7 Multidenominational Slot Machines.

Background:

Mr. Forrest William Bietz is applying for a Gaming License for Jett Gaming, LLC db at Safeway #4626, located at 2808 Vista Boulevard, Sparks, Nevada 89434. This application is filed only to retain the property’s grandfathered rights to non-restricted gaming.  Mr. Forrest William Bietz has been the Key Employee/General Manager of Terrible’s Fernley Casino since October of 2013.  Prior to him working for Terrible’s Fernley Casino, Mr. Forrest William Bietz worked as a manager for the Discovery Museum from November of 2011 until June of 2013.  From August of 2006 until March of 2011, Mr. Forrest William Bietz was the owner of Color All Technologies. Additionally, Mr. Forrest William Bietz served as an Artown Board Member from October of 2010 until October of 2013.



Analysis:

Mr. Forrest William Bietz has complied with all existing ordinances and requirements.



Alternatives:

Council may approve or disapprove this application.



Recommended Motion:

I move to approve Mr. Forrest William Bietz’s application for a Gaming License for Jett Gaming, LLC db at Safeway #4626, located at 2808 Vista Boulevard, Sparks, Nevada 89434, contingent upon the location being inspected and approved by every city, county, district, and state agency having jurisdiction over the matter and upon fingerprints being returned from the FBI that do not reflect any disqualifying arrests/convictions.
