Title: Consideration and possible approval of an application for a Gaming license for Blitzed LLC, dba Blitz Bar and Grill located at 400 S. Rock Blvd., Sparks, NV submitted Ms. Alexa Gail Carney.
Petitioner/Presenter: Ms. Alexa Gail Carney resides at 5756 Crooked Stick Way, Sparks, NV./Chief Brian Allen
Recommendation: It is recommended that Council approve Ms. Alexa Gail Carney application for a Gaming license contingent upon the location being inspected and approved by every city, county, district, and state agency having jurisdiction over the matter and upon fingerprints being returned from the FBI/CJIS that do not reflect any disqualifying arrests/convictions.
Financial Impact: : $120.00 x 10 = $1,200.00 annually. There will be 10 multi denomination coin operated machines/IGT Game Kings.

     A Business Impact Statement is not required because this is not a rule.

Background:

Ms. Alexa Gail Carney is applying for a Gaming License for Blitzed LLC, dba Blitz Bar and Grill located at 400 S. Rock Blvd., in Sparks Nevada.  Ms. Alexa Gail Carney has been the owner of Blitzed LLC since February 1st, 2014.  Prior to her owning and operating the Blitzed LLC, Ms. Alexa Gail Carney was a Restaurant Manager for Denny’s Restaurant from April, 2004 to January, 2014 located at 205 E. Nugget Avenue, Sparks, Nevada.



Analysis:

Ms. Alexa Gail Carney has complied with all existing ordinances and requirements.



Alternatives:

Council may approve or disapprove this application.

 



Recommended Motion:

I move to approve Ms. Alexa Gail Carney application for a Gaming license for Blitzed LLC, dba Blitz Bar and Grill located at 400 S. Rock Blvd., Sparks, NV contingent upon the location being inspected and approved by every city, county, district, and state agency having jurisdiction over the matter and upon fingerprints being returned from the FBI that do not reflect any disqualifying arrests/convictions.
