Title: Consideration and possible approval of an application for a Gaming license for Woody’s Entertainment, Inc. dba Bob & Lucy’s, located at 1515 Oddie Blvd., Sparks, NV, 89431,submitted by Mr. David Scott Tate.

Recommendation: It is recommended that Council approve Mr. Tate’s application for a Gaming license contingent upon the location being inspected and approved by every city, county, district, and state agency having jurisdiction over the matter and upon fingerprints being returned from the FBI/CJIS that do not reflect any disqualifying arrests/convictions.
Financial Impact: $120.00 x 15 = $1,800.00 annually. There will be fifteen, multi-denominational, upright slant slot machines.

Agenda Item Brief: Mr. David Scott Tate has submitted for consideration and possible approval, a Gaming license for Woody’s Entertainment, Inc. dba Bob & Lucy’s.
Background: Mr. David Scott Tate is applying for a restricted Gaming license for Woody’s Entertainment, LLC dba Bob & Lucy’s. Mr. Tate has been the Manager of Stillwater Gaming/WCW Corporation, a slot-route vendor located in Fallon, NV since January, 1995. From July, 1994 to December, 1994, Mr. Tate was the Manager at Palace Casinos, located in Biloxi, MS. Prior to that, Mr. Tate was a CPA at Ernst & Young, located in Phoenix, AZ. Mr. Tate obtained a Gaming license for a one-day opening of Victorian Penny Park in Sparks, NV, in November, 2011, in an effort to maintain the location’s unrestricted Gaming license. Mr. Tate was approved by the Gaming Control Board for this location; license #31942-01.
Analysis: Mr. Tate has complied with all existing ordinances and requirements.
Alternatives: Council may approve or disapprove this application.
Recommended Motion: I move to approve Mr. Tate’s application for a Gaming license for Woody’s Entertainment, Inc. dba Bob & Lucy’s, located at 1515 Oddie Blvd., Sparks, NV contingent upon the location being inspected and approved by every city, county, district, and state agency having jurisdiction over the matter and upon fingerprints being returned from the FBI that do not reflect any disqualifying arrests/convictions
